The Use of Tissue Expanders for Correction of Postburn Scars


Abstract
Although various surgical methods which include skin grafting, local, distant and free microvascular flaps for reconstruction of postburn scars have been developed, there are many disadvantages of traditional methods
, such as donor site defect,
mismatch
of color and texture, and other limited esthetic results.
Nowdays, tissue expansion becomes one of the most popular techniques in the filed of plastic surgery, especially of the correction of postburn alopecia of the scalp and wide hypertrophic scars. It was first introduced in 1957 by Dr. neumann who
attempted an ear reconstruction using a latex ballon with a valve and connector tube lying outside the skin. Later. Dr.Radovan had made it safer and more controllable.
The author used 8 expanders to treat 4 patients of postburn alopecia and a patient of postburn hypertrophic scar of the forearm during the period of September 1992 to March 1993. All cases were successfully reconstructed without a major
complication
which had led to the failure of operation. But, there were a few minor complications such as expansion of unwanted area, wound problem above the injection dome and pain on the expansion. In this paper, the author would like to present case
reports
and
discuss the detailed expansion technique indications, advantages and distadvantages, complications and overall management of tissue expansion.
